The application exists to determine whether AXIS can genuinely help you — not to filter for the best-sounding candidates. Honest answers are more useful than impressive ones. A person who is ready to do the work and says so plainly will be accepted over a person who performs readiness.

Applications are reviewed within 3 business days. If accepted, you will receive a payment link. If AXIS is not the right fit at this time, we will say so directly and explain why.

A note on this form: The questions below are diagnostic, not administrative. They are the first act of the work. How you answer them tells us more than what you answer. Vague answers, performed answers, and answers designed to seem ready are all noted — and are the most common reason for declining an application.

The basics
Not your title. What you actually spend your time on.
What brings you here
Not "I want to be better" or "I feel stuck." What specifically is not working, not changing, or not happening that you know should be. The more specific this is, the more useful it is.
Therapy, coaching, books, courses, your own attempts. What worked and what didn't — and your honest assessment of why.
You don't need to be precise. But if you have a sense of what keeps recurring — across work, relationships, self-perception — name it here as specifically as you can.
This is the most important question. Every person has a primary way of sabotaging their own development. If you know yours, name it. If you don't know it yet, say that — and describe your best guess.
Practicalities
Sessions are 60 minutes. You need to be able to reschedule at most once with 24-hour notice. A second missed session without notice is forfeited.

If accepted: you receive a payment link and the questionnaire is sent within 24 hours of payment
If not the right fit: you receive a direct explanation and, where relevant, a suggestion for what might be more appropriate right now
No money changes hands until acceptance — the application is free
